Your impact

You will provide low-barrier, trauma-informed care coordination under the Priority Populations grant for people who use drugs, particularly participants from BIPOC communities and participants impacted by the justice system. This role also requires timely documentation, clear communication, dependable continuity of services, accurate use of AIRS, and compliance with grant and confidentiality requirements.

How you will work

The Prevention Care Manager works under general supervision and exercises independent judgment within established policies. This role requires maintaining confidentiality, developing participant-centered care plans, conducting outreach, responding appropriately to urgent participant needs within the scope of the position, and providing culturally responsive advocacy with professionalism.

  • Bachelor’s degree and one year of relevant experience, or an equivalent combination of education and experience. Relevant experience may include nonprofit social services or healthcare work providing direct services with diverse communities, including people who use drugs, people impacted by the justice system, and people seeking mental health support.
  • Preferred: Experience in care management, healthcare, harm reduction, outreach, or participant navigation. Relevant lived experience and community knowledge—such as experience related to housing instability, substance use, racial or cultural inequities, LGBTQI+ communities, or justice-system involvement—are welcomed and valued.

Skills that help you succeed

  • Excellent organizational and time-management skills, with the ability to prioritize competing participant, outreach, documentation, and reporting responsibilities.
  • Strong written, verbal, and interpersonal communication skills, including active listening, professional correspondence, case conferencing, and clear communication with participants, coworkers, providers, funders, and community partners.
  • Demonstrated ability to complete accurate, objective, confidential, and timely case notes, assessments, service plans, referrals, follow-up records, and required program reports.
  • Ability to learn and work effectively in AIRS, including accurate data entry, service documentation, participant record maintenance, outcome tracking, and correction of data-quality issues.
  • Working knowledge of Microsoft Office, including Word, Excel, Outlook, and Teams, and the ability to use electronic health records and other web-based systems.
  • Ability to work independently and collaboratively, maintain professional boundaries, exercise sound judgment, and communicate emerging participant or program concerns promptly.
  • Fluency in Spanish and English preferred.
  • Demonstrated knowledge of and respect for communities affected by HIV/AIDS, chronic illness, substance use, justice-system involvement, and LGBTQ+ health inequities; commitment to cultural humility, ACR Health’s mission and values, and HIPAA requirements.

Additional requirements

  • Ability to meet the position’s scheduling requirements and provide dependable continuity of services, including timely participation in required meetings, training, participant appointments, documentation, and follow-up activities.
  • Ability to work occasional evenings and weekends and travel within the service area for outreach, appointments, meetings, training, and grant-related activities.
  • Valid New York State driver’s license and reliable access to a vehicle with current insurance for work-related travel, training, outreach, and occasional client transport (personal vehicle ownership is not required, but consistent access is necessary).

What you will do

  • Maintain confidentiality in accordance with Article 27-F, HIPAA, ACR Health policies and procedures, the HIV Confidentiality Law, grant requirements, and other applicable regulations; safeguard participant and agency data.
  • Conduct intakes, eligibility reviews, strengths-and-needs assessments, social determinants of health screenings, and ongoing reassessments; develop participant-centered service plans with measurable goals.
  • Provide trauma-informed, culturally responsive care coordination and advocacy related to housing, food, healthcare, behavioral health, MOUD, overdose prevention, benefits, transportation, and legal or reentry needs.
  • Engage Priority Populations grant participants through low-barrier office-based and community-based services, including outreach with people who use drugs, particularly community members from BIPOC communities and people impacted by the justice system, as well as others seeking support in accessing care and services.
  • Facilitate warm handoffs and referrals to culturally responsive providers; confirm service connection, help address access barriers, and offer timely follow-up when participants are unable to attend appointments or experience a gap in services.
  • Maintain consistent, professional communication with participants, coworkers, providers, justice-system contacts, and community partners, and promptly elevate urgent safety, service, compliance, or participant concerns to the supervisor.
  • Enter all participant contacts, services, assessments, referrals, outcomes, and follow-up activities accurately and within required timelines in AIRS and any other designated record system.
  • Maintain complete, objective, and audit-ready participant records; review work for accuracy, reconcile missing or inconsistent information, and correct documentation or data-quality issues promptly.
  • Track grant deliverables, participant demographics, service utilization, referrals, linkage outcomes, and other required performance measures; prepare accurate monthly, quarterly, annual, and ad hoc reports by established deadlines.
  • Use Microsoft Word, Excel, Outlook, Teams, AIRS, electronic health records, and other approved systems to manage documentation, communication, schedules, data tracking, and reporting.
  • Participate in case conferencing, multidisciplinary coordination, program meetings, quality-improvement activities, chart reviews, grant monitoring, and required training.
  • Maintain participant contact to support self-identified goals and access to services; provide regular reassessment, timely support for urgent needs within the scope of the role, and participant-centered transition or discharge planning as appropriate.
  • Authorize or arrange approved supportive and medical services within program guidelines and advocate to reduce structural and practical barriers to care.
