Position Summary: The Prevention Care Manager provides low-barrier, trauma-informed care coordination for BIPOC and justice-involved people who use drugs. Using a harm reduction approach, the PCM supports participants in accessing healthcare, behavioral health, MOUD, support services, benefits, and entitlements through needs assessments, action plans, advocacy, and follow-up to improve health outcomes and reduce overdose risk.

Authority and Independence

The Prevention Care Manager works under general supervision and exercises independent judgment within established policies. This role requires maintaining confidentiality, developing care plans, conducting outreach/crisis intervention, and providing culturally responsive advocacy with professionalism.

Minimum Qualifications

 Education and Experience

  • Bachelor’s degree + 1 year experience OR equivalent combination; qualifying experience in nonprofit social services/healthcare providing direct services to diverse populations (PWUD, justice-involved, mental health).
  • Preferred: Case management/healthcare experience; lived experience (homelessness, substance use, BIPOC, LGBTQI+).

Skills and Competencies

  • Excellent organizational/time management; computer proficiency (Microsoft Office; EHR preferred).
  • Fluency in Spanish/English preferred.
  • Sensitivity to HIV/AIDS, chronic illness, LGBTQ+ issues; commitment to ACR Health mission/values and HIPAA.

Other Requirements 

  • Valid New York State driver’s license and reliable access to a vehicle with current insurance for work-related travel, training, outreach, and occasional client transport (personal vehicle ownership is not required but consistent access is necessary).

Primary Responsibilities

  • Maintain confidentiality per Article 27-F, HIPAA, ACR Health policies/regulations; protect agency data/HIV Confidentiality Law.
  • Conduct intakes, risk assessments, SDOH screenings; develop client-centered care plans with providers.
  • Provide trauma-informed care coordination/advocacy for housing, food, healthcare, behavioral health, MOUD, legal navigation.
  • Collaborate on community outreach; facilitate warm handoffs/referrals to culturally responsive providers.
  • Maintain contact for motivation/services acquisition; conduct regular reassessments/crisis intervention.
  • Maintain accurate stats/records; prepare monthly/program reports; submit documentation timely.
  • Authorize/arrange support/medical services; advocate to reduce service barriers.
  • Complete 24+ hours annual training; coordinate case conferencing with partners/justice system/internal staff.
  • Perform other duties as assigned by Health Homes Manager, Director of Care Management Services, CPO, or CEO.

FLSA Status:  Non-Exempt

Location:  Syracuse Office  Hours per week:  35   Work Schedule:  Full Time in Office, occasional evenings and weekends may be required for meetings, workshops, outreach, and events.
