Salary: $24.00-$26.00 per Hour

ACR Health is seeking a compassionate, motivated, and community-minded Linkage Specialist to join our Harm Reduction & Prevention Services team. This position plays a vital role in connecting people to healthcare, harm reduction services, treatment programs, and community resources through mobile outreach and participant-centered service delivery.
The successful candidate will work directly with individuals experiencing substance use, behavioral health needs, housing instability, chronic health conditions, economic hardship, and other social determinants of health. This is an exciting opportunity to improve health outcomes, reduce challenges accessing services, and support people in achieving their self-identified goals.
What You’ll Do
Build Meaningful Connections
- Engage participants through mobile outreach services and community-based programs.
- Conduct assessments, needs and safety assessments, and service planning activities.
- Provide supportive, participant-centered case management and navigation services.
- Deliver education on harm reduction, overdose prevention, healthcare access, and available community resources.
- Build trusting relationships that promote long-term engagement and positive outcomes.
Connect People to Care
- Facilitate access to Medication for Opioid Use Disorder (MOUD), Hepatitis C treatment, behavioral health services, primary care, and other healthcare support.
- Coordinate referrals and collaborate with providers, treatment programs, and community partners.
- Assist participants with obtaining insurance coverage, benefits, identification documents, and other essential resources.
- Advocate alongside participants as they navigate healthcare and community service systems.
Support Mobile Outreach Services
- Travel throughout the service region on ACR Health’s mobile outreach unit three to four days per week.
- Engage community members seeking services through mobile outreach and service delivery.
- Provide information about available programs, services, and resources.
- Maintain participant records and support continuity of care for people accessing services.
Manage Data and Program Operations
- Document services accurately and timely within agency systems.
- Track participant outcomes and program performance measures.
- Assist with quality improvement initiatives, audits, and reporting requirements.
Strengthen Community Partnerships
- Build relationships with healthcare providers, community organizations, and referral agencies.
- Participate in outreach events, trainings, and educational activities.
- Serve as a positive ambassador for ACR Health throughout the community.
What We’re Looking For
Required Qualifications
- Minimum one (1) year of experience in social services, healthcare, behavioral health, harm reduction, or another human services setting.
- Experience providing direct support to people experiencing complex social, behavioral health, or healthcare needs.
- Strong communication, documentation, and organizational skills.
- Ability to work independently while managing multiple priorities.
- Proficiency with Microsoft Office, Microsoft Teams, SharePoint, and electronic data systems.
- Valid New York State driver’s license.
- Reliable transportation and willingness to travel throughout the service region.
- Ability to work a regular schedule of Tuesday through Saturday, 11:00 a.m. to 7:00 p.m.
Preferred Qualifications
- Experience working with individuals experiencing substance use, behavioral health needs, housing instability, chronic health conditions, legal system involvement, or economic hardship.
- Experience delivering low-threshold, culturally responsive, and stigma-free services.
- Knowledge of harm reduction principles, overdose prevention, HIV/AIDS, Hepatitis C, behavioral health, chronic health conditions, and LGBTQ+ health.
- Lived experience related to substance use, recovery, overdose, and/or legal system involvement is valued, and applicants with such experience are strongly encouraged to apply.
