ACR Health is seeking a compassionate, organized, and motivated Housing Coordinator II to oversee the daily operations of our Health Home Supportive Housing (HHSH) program. This role is ideal for someone who is passionate about helping individuals and families facing housing instability while supporting positive health outcomes for people living with chronic illnesses.

As a Housing Coordinator II, you will provide program leadership, support staff development, foster relationships with community partners and landlords, and help individuals and families access and maintain stable housing. Your work will directly support ACR Health’s mission of improving health outcomes and strengthening our communities.

What You’ll Do:
Responsibilities include, but are not limited to:
Program Leadership & Oversight
• Provide oversight of the HHSH program to ensure services align with agency policies, mission, and contract requirements.
• Supervise, support, and provide guidance to program staff.
• Assist with staff training, performance management, and professional development.
• Ensure compliance with agency policies and applicable regulations, including HIPAA, Article 27-F, and related confidentiality requirements.
• Client & Housing Services
Client & Housing Services
• Conduct and oversee Housing Risk Assessments, Housing Inspections, and other required assessments.
• Assist clients in obtaining and maintaining safe, stable, and affordable housing.
• Coordinate referrals, applications, housing resources, and supportive services.
• Provide crisis intervention and problem-solving support for clients and staff.
• Complete required documentation related to service delivery, home visits, case conferences, and other client contacts.
Community Partnerships & Advocacy
• Develop and maintain relationships with landlords, property managers, community organizations, and service providers.
• Advocate for clients and assist with resolving housing-related challenges.
• Collaborate with internal and external partners to enhance service delivery and housing outcomes.
• Represent ACR Health at community meetings, conferences, and professional networking opportunities.
Compliance, Data & Reporting
• Monitor program outcomes and performance measures.
• Ensure client records, reports, and statistical data are completed accurately and timely.
• Maintain knowledge of housing regulations, assistance programs, and best practices.
• Assist with grant reporting, funding applications, and other program sustainability efforts.
• Facilitate staff meetings, case conferences, and multidisciplinary discussions.

What We’re Looking For:
Minimum Qualifications
Education
• Bachelor’s degree in social work, Public Health, Human Services, or a related field preferred.
• Equivalent combinations of education and experience will be considered.
Experience
• At least two years of experience in HIV/AIDS services, behavioral health, substance use services, housing support, or other chronic illness-related programs preferred.
• Previous supervisory experience and experience working with complex populations strongly preferred.
Other Requirements
• Valid New York State driver’s license.
• Reliable access to an insured vehicle that meets DMV requirements for work-related travel.
• Ability to travel throughout ACR Health’s service area as needed.
Skills and Competencies
• Ability to develop partnerships and foster collaboration among landlords, housing providers, healthcare professionals, and community stakeholders.
• Ability to build and maintain positive relationships with clients, landlords, community agencies, and funding partners.
• Ability to provide leadership, coaching, and support to program staff.
