Housing Coordinator I

Reports to: Director of Family, Education & Support Services

Location: Syracuse, NY

Schedule: Full-Time (35 Hours/Week)

FLSA Status: Exempt

Salary: $27.04 – $28.57 hourly

ACR Health is seeking a compassionate, organized, and motivated Housing Coordinator to support individuals and families enrolled in our Housing Opportunities for Persons with AIDS (HOPWA) program.

This position is ideal for someone who is passionate about helping people overcome housing barriers, maintain stable housing, and access the resources they need to improve their overall health and well-being. As a Housing Coordinator I, you will work directly with participants, landlords, healthcare providers, and community organizations to promote housing stability, self-sufficiency, and positive outcomes.

Your work will directly support ACR Health’s mission of improving health outcomes and strengthening communities throughout Central New York.

What You’ll Do

Responsibilities include, but are not limited to:

 Client Housing Services

  • Conduct participant intakes, screenings, assessments, and eligibility reviews.
  • Complete housing assessments and individualized housing stability plans.
  • Assist participants in obtaining and maintaining safe, stable, and affordable housing.
  • Coordinate housing applications, recertifications, and required program documentation.
  • Conduct home visits and housing inspections as required.
  • Provide support, problem-solving, and crisis intervention related to housing needs.

Housing Stability & Advocacy

  • Educate participants about tenant rights and responsibilities.
  • Assist participants in developing independent living and housing retention skills.
  • Communicate with landlords and property managers to address housing-related concerns.
  • Advocate on behalf of participants to reduce barriers to housing stability.
  • Coordinate referrals for benefits, transportation, food security, healthcare, and other supportive services.

Care Coordination & Community Collaboration

  • Collaborate with healthcare providers, case managers, and community organizations.
  • Participate in multidisciplinary case conferences and service planning activities.
  • Maintain positive working relationships with landlords, housing providers, and community partners.
  • Represent ACR Health at community meetings, trainings, and events as assigned.

Compliance & Documentation

  • Maintain accurate participant records and documentation.
  • Verify eligibility, income, and required program documentation.
  • Complete required reports, data entry, and outcome tracking activities.
  • Ensure compliance with HOPWA regulations, HIPAA, Article 27-F, and agency policies.

We’re Looking For

Education

Bachelor’s degree in human services, Social Work, Public Health, Psychology, Education, or a related field preferred. Equivalent combinations of education and experience will be considered.

Experience

  • One to two years of experience in housing services, case management, homelessness prevention, HIV/AIDS services, behavioral health, substance use services, or related human services programs preferred.
  • Experience working with vulnerable populations is strongly preferred.

Skills & Competencies

  • Strong communication, advocacy, and relationship-building skills.
  • Ability to work effectively with diverse populations.
  • Strong organizational and time-management abilities.
  • Ability to maintain confidentiality and professional boundaries.
  • Experience coordinating services and navigating community resources.
  • Proficiency with Microsoft Office and electronic recordkeeping systems.

Other Requirements

  • Valid New York State driver’s license.
  • Reliable access to an insured vehicle for work-related travel.
  • Ability to travel throughout ACR Health’s service area.
