​ACR Health’s Support Services are designed to provide comprehensive, compassionate assistance to individuals and families facing chronic health conditions, including HIV/AIDS, mental health challenges, and substance use disorders. Our goal is to empower clients to achieve stability, independence, and improved health outcomes through personalized care and support.

Health Homes Care Management

ACR Health’s Health Homes Care Management program offers comprehensive, individualized support to Medicaid-eligible individuals living with chronic physical and behavioral health conditions. Our dedicated care managers collaborate closely with clients to coordinate medical, behavioral, and social services, aiming to improve health outcomes and enhance quality of life.​ By integrating a wide range of services, ACR Health’s Health Homes Care Management program strives to reduce hospitalizations, prevent emergency room visits, and support clients in achieving optimal health and independence.

Developing tailored care plans that address each client’s unique health needs and goals.​

Facilitating access to primary care, mental health services, and substance use treatment.​

Assisting with housing stability, legal aid, financial benefits, nutrition education, and transportation to medical appointments.

Regularly reviewing and adjusting care plans to ensure they remain aligned with clients’ evolving needs.​

Housing Assistance

ACR Health helps clients access and maintain safe, stable, and affordable housing—because housing is healthcare. Our Housing Assistance Program supports people living with chronic conditions such as HIV/AIDS or substance use disorders by removing barriers to shelter and security.

ACR Health provides short term curricula-based one-on-one and group-based health education designed to reduce health risks, promote safer behaviors, and empower individuals to take control of their health. Education and risk reduction services are structured to enhance the knowledge base, health literacy, and self-efficacy of people living with HIV in accessing and maintaining HIV medical services and staying healthy. Services are trauma-informed, inclusive, and tailored to each individual’s needs.

Medical Transportation

Medical Transportation is for people living with HIV/AIDS who need help with transportation to get to medical and other services that help them maintain their health.

Medical Transportation can be used for qualifying apointments:

Transportation assistance may be in the form of taxi rides, bus passes, and gas cards.

Medical Transportation is a payer of last resort, all other sources of transportation must be tried and denied before a request is submitted to Medical Transportation.

Nutrition Health Education

Nutrition Education is a service available for people living with HIV/AIDS who want to learn how the foods they eat will affect their overall health.
