ACR Health is seeking a compassionate, community-focused, and motivated Syracuse-Oswego SEP Program Associate to support harm reduction services and community outreach throughout Onondaga and Oswego Counties.
This role is ideal for someone who enjoys building relationships, working independently in community settings, and connecting individuals to resources that improve health and well-being. The Syracuse-Oswego SEP Program Associate plays a key role in expanding access to harm reduction services through outreach, participant engagement, overdose prevention initiatives, community partnerships, and Syringe Exchange Program (SEP) operations.
A significant portion of this position involves field-based work in Oswego County, including community outreach, relationship-building with local partners, participation in Ride Along services, and support for post-overdose engagement activities. The successful candidate will be comfortable working independently while serving as a visible and trusted ACR Health representative in rural and underserved communities.
What You’ll Do
Responsibilities include, but are not limited to:
Community Outreach & Engagement
• Conduct outreach and engagement activities throughout Onondaga and Oswego Counties, with a strong focus on rural and underserved communities.
• Develop and maintain positive relationships with community partners, healthcare providers, local organizations, first responders, and other stakeholders.
• Participate in Ride Along services, post-overdose engagement activities, and other outreach initiatives.
• Represent ACR Health at community meetings, health fairs, trainings, presentations, and networking events.
• Identify opportunities to increase awareness of and access to harm reduction services.
Participant Services
• Provide participant-centered, stigma-free harm reduction services.
• Enroll participants and support ongoing engagement in SEP services.
• Provide overdose prevention education and naloxone distribution.
• Connect participants to healthcare, HIV/HCV/STI services, substance use treatment and MOUD, behavioral health services, housing resources, benefits, and other community supports.
• Recruit participants and peer educators for program activities.
SEP Operations & Inventory Management
• Support daily Syringe Exchange Program operations and service delivery.
• Promote access to sterile injection equipment and safe sharps disposal.
• Maintain inventory of harm reduction, overdose prevention, and SEP supplies.
• Coordinate supply distribution for outreach activities and service locations.
• Assist with drug-checking services and participant education as trained and assigned.
Documentation & Compliance
• Maintain accurate and timely participant records, service documentation, referrals, and outreach activities.
• Complete required reports and data collection activities.
• Ensure compliance with agency policies, confidentiality requirements, and applicable regulations, including HIPAA and Article 27-F.
• Maintain professional standards and represent ACR Health positively in the community.
What We’re Looking For
Minimum Qualifications
Education
• High School Diploma or GED required.
• Relevant education in Human Services, Public Health, Social Work, Healthcare, or a related field is preferred.
Experience
• At least one year of experience in social services, public health, healthcare, outreach, harm reduction, or a related field preferred.
• Experience working with individuals experiencing substance use, housing instability, mental health challenges, justice involvement, or other barriers to care is strongly preferred.
• Experience conducting outreach and community engagement activities preferred.
• Experience working with LGBTQI+, TGNCNB, BIPOC, and other historically underserved communities preferred.
Other Requirements
• Valid New York State driver’s license.
• Reliable access to an insured vehicle for work-related travel.
• Ability to travel regularly throughout Onondaga and Oswego Counties.
• Ability to occasionally work evenings and weekends for outreach activities, community events, and meetings.
