Position Summary: The HCV Coordinator engages and supports individuals who use substances by connecting them to HCV screening, testing, treatment, and care navigation services. Working closely with SEP and Health Hub participants, this role identifies individuals who may be unaware of their HCV status, facilitates access to care, maintains a caseload of clients with HCV, and helps address barriers that may affect treatment engagement and completion.

Authority and Independence: The HCV Coordinator works under the general supervision of the HCV Program Manager and exercises independent judgment in participant engagement, care navigation, outreach, education, and service coordination activities. The position requires maintaining strict confidentiality, building collaborative relationships with providers and community partners, implementing low-threshold and harm reduction service models, and delivering culturally responsive, stigma-free services to diverse populations.

Minimum Qualifications

Education and Experience

  • Bachelor’s degree and one (1) year of experience working in a nonprofit or healthcare setting; OR an equivalent combination of education and relevant experience.
  • At least one (1) year of experience providing stigma-free, non-judgmental, and culturally sensitive services.
  • Basic knowledge of Hepatitis C Virus (HCV), HIV, sexually transmitted infections (STIs), and harm reduction principles is strongly preferred.
  • Experience documenting services, maintaining case notes, and managing program records for funded activities.
  • Experience working with individuals impacted by substance use, chronic illness, housing instability, or other barriers to care is preferred.
  • Lived or living experience with HCV and/or substance use is valued and applicants with such experience are encouraged to apply.

  • Demonstrated commitment to cultural humility and sensitivity regarding HIV, chronic illness, harm reduction, LGBTQ+, BIPOC, and substance-using communities.

Other Requirements

  • Valid New York State driver’s license and reliable access to a vehicle with current insurance for work-related travel, training, outreach, and occasional client transportation. Personal vehicle ownership is not required, but consistent access is necessary.
  • Ability to work occasional evenings and weekends for meetings, community outreach events, trainings, workshops, conferences, and seminars.

Primary Responsibilities

  • Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F HIV Confidentiality Law, 42 CFR Part 2 Substance Use regulations, HIPAA, ACR Health policies and procedures, and other applicable regulations.
  • Identify individuals engaged in active substance use who do not know their HCV status and connect them with screening, testing, referral, and treatment services.
  • Conduct in-reach activities within the Syringe Exchange Program (SEP) and Health Hub to identify participants who may benefit from HCV testing, education, and treatment support.
  • Promote harm reduction strategies before, during, and after HCV treatment to reduce transmission risk and prevent reinfection.
  • Enroll eligible participants into HCV Navigation services and support engagement throughout the continuum of care, including sustained virologic response (SVR) confirmation.
  • Maintain a caseload of participants living with HCV and provide ongoing care navigation, supportive services, and treatment support.
  • Conduct assessments and care planning activities, including risk assessments, treatment readiness discussions, counseling, goal setting, and service planning.
  • Assist participants in overcoming barriers to care, including transportation, communication access, housing instability, healthcare navigation, and other social determinants of health.
  • Provide adherence counseling and support participants in maintaining engagement throughout the treatment process.
  • Facilitate linkage and referrals to medical care, primary care providers, HCV treatment programs, substance use treatment services, Medication for Opioid Use Disorder (MOUD) programs, peer services, support groups, and other community resources.
  • Implement designated program interventions and recruitment strategies outlined in applicable work plans, contracts, and funding requirements.
  • Maintain accurate and complete client records, service documentation, and case notes in compliance with agency, contract, and regulatory requirements.
  • Collect, analyze, and report program data as required by funding sources, regulatory agencies, and internal reporting needs.
  • Prepare and submit required monthly, quarterly, annual, and ad hoc reports within established timelines.
  • Develop and maintain collaborative relationships with healthcare providers, treatment programs, and community-based organizations to enhance access to care.
  • Attend community provider meetings, advisory boards, workshops, coalitions, and other forums focused on the needs of priority populations.
  • Represent ACR Health in a professional manner and promote agency services throughout the service region.
  • Participate as a culturally competent member of the department and agency, demonstrating respect for the beliefs, values, behaviors, and needs of diverse communities.
  • Attend required training and complete a minimum of forty (40) hours of professional development annually.
  • Perform other duties as assigned by the HCV Program Manager, Chief Program Officer, and/or their Chief Executive Officer.

FLSA Status: Exempt. Office  Hours per week:  35   Work Schedule:  Full Time in Office, occasional evenings and weekends may be required for meetings, workshops, outreach, and events.
