Job Title:  Outreach Coordinator

Position Summary: The Outreach Coordinator will oversee outreach teams within as-signed regions, ensuring effective service delivery and operational coordination. Responsibilities include supervising staff, managing outreach schedules, reviewing data and reports, and collaborating with other coordinators and the program manager to support services within assigned region. The coordinator will also assist with coverage as needed and per-form client intakes and linkages to care and community resources. The position follows a Tuesday to Saturday schedule, including evenings and weekends.

Authority and Independence: The Outreach Coordinator works under general supervision, exercising sound judgment within established agency policies and procedures. The role requires independent initiative, strong organizational skills, and strict adherence to confidentiality while managing multiple priorities and ensuring program effectiveness.

Minimum Qualifications

 Education and Experience

  • Diploma or GED with at least two (2) years of experience in a nonprofit social service agency providing direct client services, or a comparable combination of education and experience.
  • At least one (1) year of experience in a social service or healthcare setting providing direct services to individuals who use drugs and members of marginalized communities (including BIPOC and LGBTQI+ individuals) is highly desirable.
  • Experience working with diverse populations such as people who use drugs (PWUD), individuals with justice involvement, mental health challenges, or those experiencing homelessness is highly preferred.
  • Lived experience with homelessness, substance use, justice involvement, and/or overdose is preferred.

 Skills and Competencies

  • Strong understanding of issues related to HIV, chronic illness, substance use, overdose, and behavioral health.
  • Demonstrated commitment to harm reduction principles, health equity, and confidentiality standards (HIPAA and Article 27F).
  • Commitment to ACR Health’s mission, vision, and values.
  • Excellent communication, coordination, and documentation skills.
  • Ability to deliver stigma-free, culturally competent, and non-judgmental services.
  • Ability to collaborate effectively with internal staff, community partners, and referral agencies.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams, SharePoint, PowerPoint).

 Other Requirements

  • Valid New York State driver’s license and reliable access to a vehicle with current insurance for work-related travel, training, outreach, and occasional client transport (personal vehicle ownership not required but consistent access is necessary).
  • Ability to work Tuesday–Saturday 11am–7pm schedule, including evenings, weekends, and non-traditional hours as required.
  • Bilingual (Spanish/English) preferred.

Primary Responsibilities

  • Supervise outreach staff and coordinate outreach activities for assigned teams, providing guidance, support, and oversight to ensure effective service delivery and achievement of program deliverables.
  • Incorporate best practices regarding confidentiality into all job duties and communications consistent with Article 27-F, 42 CFR, HIPAA, ACR Health policies and procedures, and other applicable regulations.
  • Oversee and support referral processes, ensuring participants are connected to internal and external services including harm reduction resources, testing services, risk reduction counseling, MOUD/MAT providers, and social service agencies.
  • Maintain accurate client and service data in AIRS and other systems, ensuring timely completion of required reports.
  • Develop and maintain strong relationships with community partners and referral networks.
  • Collaborate with the Program Manager and staff to track performance indicators, coordinate schedules, and oversee field operations.
  • Conduct outreach, client engagement, recruitment, screenings, referrals, and linkages to care.
  • Engage priority populations through community-based outreach efforts and harm reduction education.
  • Facilitate connections to mental health, prevention, primary care, specialty care, and substance use treatment services.
  • Represent ACR Health professionally and maintain a culturally competent presence in all interactions.
  • Participate in staff meetings, training, and professional development opportunities.
  • Perform other duties as assigned by program leadership or executive management.

Location:  Syracuse, Utica, Watertown Office  Hours per week:  35   Work Schedule: Full-time in office, Tuesday–Saturday 11am–7pm; includes evenings, weekends, and non-traditional hours for meetings, workshops, outreach, and events as required.
