 Position Summary: The Outreach Coordinator supports the Syringe Exchange Program (SEP) by planning, coordinating, and delivering harm reduction services to people who use drugs. This role focuses on building trust within the community, reducing the spread of infectious diseases, preventing overdose deaths, and connecting participants to health and social services

Authority and Independence

The Outreach Coordinator works under general supervision, exercising sound judgment within established agency policies and procedures. The position requires independent initiative, confidentiality, and organizational skills to manage multiple tasks and ensure service delivery aligns with agency standards and community needs.

Minimum Qualifications

Education and Experience

  • At least two years of experience working in a social service or healthcare agency providing direct services to People Who Use Drugs (PWUD) and other diverse populations (e.g., people of color, LGBTQI+ individuals, people with behavioral health needs, or those experiencing homelessness).
  • Experience providing stigma-free, nonjudgmental, and culturally competent services.

Skills and Competencies

  • Knowledge and acceptance of harm reduction principles and a strong commitment to health equity.
  • Sensitivity to issues related to HIV/AIDS, substance use, and LGBTQ+ populations, with alignment to ACR Health’s mission and values.
  • Strong communication, coordination, and documentation skills.
  • High attention to detail and accuracy in data entry.
  • Ability to work independently and collaboratively with staff, community partners, and clients.

Other Requirements 

Valid New York State driver’s license and reliable access to a vehicle with current insurance for work-related travel, training, outreach, and occasional client transport (personal vehicle ownership is not required but consistent access is necessary).

Primary Responsibilities

  • Maintain confidentiality in accordance with Article 27-F, HIPAA, and ACR Health policies.
  • Coordinate SEP outreach and engagement efforts across rural areas of Cayuga, Oswego, and Oneida Counties.
  • Plan and deliver evidence-based harm reduction services including sterile syringe access, safer injection education, naloxone training, overdose prevention kits, and related interventions.
  • Assist with SEP mobile service delivery in Oswego, including Ride About and special arrangements.
  • Supervise SEP Outreach Specialists and support staff as assigned.
  • Support initiatives to improve access to overdose prevention services, including drug checking and linkage to MOUD.
  • Ensure accurate and timely data entry into the AIDS Institute Reporting System (AIRS).
  • Maintain complete and accurate client and service records per agency protocols.
  • Prepare required reports (monthly, quarterly, and annual) as directed.
  • Establish and maintain community partnerships and referral relationships.
  • Participate in community outreach, education, and professional development (a minimum of 24 hours annually).
  • Maintain agency equipment and vehicles responsibly, performing basic safety checks.
  • Represent ACR Health respectfully and professionally in all community and agency interactions.
  • Perform other duties as assigned by Program Management, the Chief Program Officer, and/or the Chief Executive Officer.

FLSA Status:  Non-Exempt

Location:  Syracuse Office  Hours per week:  35   Work Schedule:  Full Time in Office, occasional evenings and weekends may be required for meetings, workshops, outreach, and events.
