Classification: Non-Exempt, full-time, thirty-five-hour (35.0) week; occasional evenings and weekends required for meetings, workshops, conferences, and seminars.

Position Summary: Provide peer support to individuals living with HIV/AIDS and their families to promote increased client involvement in their healthcare.
MINIMUM QUALIFICATIONS
- Bachelor’s degree in human services, social work, education, or a related field with at least two years of experience working with qualifying populations (HIV/AIDS, SMI, homeless) required with one (1) to two (2) years of experience developing and facilitating health education in a group setting. Or an equivalent combination of education and work experience.
- Possess a clear understanding of community level work and the importance of collaborating and coordinating with other individuals and organizations.
- Work collaboratively with staff to plan and implement client-centered support.
- Complete, maintain and submit all required documentation and forms within designated time frames.
- Detail oriented with strong organizational skills.
- Effective communication & documentation skills, cultural & linguistic competence, as well as experience delivering stigma-free, non-judgmental, and culturally responsive services is required.
- Computer literacy, including email management and Microsoft Suite use required. Database experience preferred.
- Sensitivity to HIV/AIDS, chronic illnesses, LGBTQI+ issues, BIPOC populations, and a strong commitment to the mission, vision, and values of ACR Health are essential.
- Access to a reliable automobile with insurance and a valid NYS driver’s license that will pass DMV screening is required.
- Ability to travel throughout ACR Health’s Ryan White Part B service region, including eleven counties in Central New York.
PRIMARY RESPONSIBILITIES
- Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F and HIPAA, ACR Health policies and procedures and other applicable regulations. Protect agency data in accordance with confidentiality procedures and protocols. Observe and abide by the HIV Confidentiality Law and HIPAA.
- Provide individual and group education level activities to address medical and health related education to people living with HIV/AIDS in our eleven-county region. The Support Specialist will increase clients’ knowledge of and participation in their health care and address HIV, STI and Hepatitis C prevention
- Conduct targeted outreach to PLWH/A needing health education and accept referrals from health and human service providers.
- Document progress notes that summarize client progress, accomplishments, and services rendered.
- Responsible for documenting each session and ensuring quality AIRS data-entry.
- Develop education curricula and activities with approval from Manager.
- Monitors the appropriateness of the food and meal service provider to client.
- Utilize evidence-based interventions and follow established curricula as put forth by NYSDOH AIDS Institute.
- Collaborate with the CIRC in assessing client needs, service plan development to ensure that goals and objectives are included and participate in case conferences.
- Maintain accurate client records that document attendance, participation, progress, accomplishments, and additional needs in AIRS and chart.
- Ensure client adherence to following protocol for FB/HDM.
- Facilitate group and individual education sessions.
- Establish connections and communicate frequently with community partners and providers and makes referrals, as necessary.
- Coordinate activities with the Peer Navigator.
- Communicate and collaborate with AIDS Institute staff and respond timely and appropriately to all requests.
- Maintain complete and accurate statistical information; prepare all required monthly and program reports as assigned; ensure thorough, up to date and complete client records.
- Maintain working knowledge of Ryan White Part B Program regulations, policies, and procedures.
- Attends all AIDS Institute and ACR Health required meetings and trainings.
- Perform other duties as assigned by the Ryan White Part B Program Manager, Director of Family, Education & Support Services, Chief Program Officer and/or Chief Executive Director.
