Housing Support Services

 

For over a decade, ACR Health has provided Housing Support Services to individuals and their families throughout a 9 county region which includes; Cayuga, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, and St. Lawrence County. The goal of the program is to reduce housing vulnerability and its associated negative health effects by assisting individuals and their families in obtaining and/or maintaining permanent housing.  We currently provide a variety of services to eligible participants, and utilize the many other support services ACR Health offers, such as Case Management and Employment Services, to assure all participants receive a complete continuum of care, successfully maintain stable housing, and achieve self-sufficiency. 

 

Housing Support Services provides:

 

Housing Placement Assistance and Referral Services:

 

  • Assist eligible clients in connecting with available housing and identifying housing resources.
  • Assist eligible clients with applying for rental subsidies and other financial assistance needed to assist with their housing placement.
  • Assists participating clients by advocating on their behalf with landlords and property owners.
  • Refers participating clients to additional supportive services.
  • Maintains contact with participating clients for 90 days to assure housing is secure and maintained.

 

 

Non-Intensive Housing Case Management

 

  • Provide case management services to increase consumer’s capacity to live independently and self sufficiently.
  • Assist eligible clients in sustaining their stability by meeting immediate health
    and psychosocial needs.
  • Refers participating clients to additional supportive services.
  • Advocate for participating clients to receive safe and appropriate housing, and educate the clients on how to advocate for themselves.

 

 

Skills Development for Health and Independent Living

 

  • Provide individual and group sessions on a variety of topics including:
    • Tenants Rights and Responsibilities
    • Landlord and Tenant Relations
    • Money Management and Budgeting
    • Treatment Adherence
  • Improve client’s capacity to live independently in permanent housing.

 

 

Housing Retention Services

 

  • Assist eligible clients in maintaining stable housing to assure long term housing security.
  • Assists participating clients by advocating on their behalf with landlords and property owners.
  • Refer participating clients to additional supportive services as needed.
  • Assists eligible clients in sustaining their stability by meeting immediate health and psychosocial needs.
  • Provide case management services to increase consumer’s capacity to live independently and self sufficiently.
